ONCE SELECTED TO BE A NSAYN PIN-UP GIRL:
YOU MUST ALSO FILL OUT AND SUBMIT A PHOTO RELEASE AND MODEL RELEASE FORM.
ALONG WITH YOUR APPLICATION AND PHOTOS, PLEASE SEND US TWO FORMS OF I.D. YOU CAN SIMPLY SCAN AND EMAIL us your two forms of I.D. or send them to us via postal mail to the address listed below. The following will be the only forms of Identification acceptable:
A current and valid State or Local Issued Drivers License or I.D., a passport, a copy of your birth certificate, your social security card, a National Insurance Card, a military identification card, or a Government issued I.D. Please make sure any form of I.D. is current and please send front and back copies of any of the acceptable forms of I.D.
Photos of your two forms of I.D. will only be accepted on a case by case basis and only if the photos are very clear and legible.
***Please scan each form of I.D. front and back as one file or send them to us in a .zip file.
